    Researchers from the Czech Republic recently found that Exjade is an effective and safe treatment for the removal of excess iron in lower-risk myelodysplastic syndromes patients. Additionally, their analyses indicate that Exjade may be more effective than Ferriprox in lower-risk patients who have high iron levels in the blood.

    Dr. Jaroslav Cermak, lead author of the study, presented the results at the 2011 European Hematology Association (EHA) Congress in London.

    Patients with myelodysplastic syndromes (MDS) may receive red blood cell transfusions to increase low red blood cell levels. Excess iron accumulates in the blood with repeated transfusions, which can result in fatigue and abdominal discomfort. In more serious cases, excess iron may lead to heart failure, diabetes, muscle cramps, and a weakened immune system.

    In order to remove excess iron, patients may undergo iron chelation therapy, in which they are treated with organic compounds that bind free iron in the blood and remove it from the body.

    In the study, 65 MDS patients with less than 10 percent of immature blood cells underwent iron chelation therapy with the oral iron chelator Exjade (deferasirox). The daily dosage ranged from 10 to 40 mg/kg. The median length of treatment was 13.7 months.

    The researchers found that Exjade maintained or reduced iron deposits in 83 percent of patients.

    The most common side effects were gastrointestinal tract complications (34 percent), which restricted the researchers from increasing the daily dosage of the drug. Six percent of patients discontinued Exjade treatment due to side effects.

    The researchers also compared their results to the results from a previous study evaluating the efficacy of Ferriprox (deferiprone) in 48 patients with the same subtype of MDS.

    They found that Exjade and Ferriprox were similarly effective in patients with low iron levels in the blood ( 2000 mg L). However, Exjade was significantly more effective than Ferriprox in patients with high iron levels in the blood ( 2000 mg L). Of the patients with high iron levels, 80 percent responded to Exjade as compared to 46 percent of patients who received Ferriprox.

    The rate of side effects was comparable for both treatments (57 percent for Exjade versus 63 percent for Ferriprox). Exjade-related side effects generally produced mild and mostly temporary symptoms. In addition, Exjade did not affect blood cell levels.

    By contrast, Ferriprox treatment resulted in low levels of granulocytes (a certain type of white blood cell) in 17 percent of patients, and 29 percent of patients discontinued treatment due to treatment-related side effects.
